It is the student's responsibility to inform and update Health Services regarding his or her health insurance status. The student (and/or family) is responsible for any pre-certification or pre-authorization required by an HMO/PPO or other insurance coverage for medical care while at St. Bonaventure.
Medications unavailable at Health Services, services and referrals off campus (emergency room, x-ray, lab work, other physicians, etc.) are the direct financial responsibility of the student.
No written medical excuses are issued by Health Services. The student is responsible for notifying his/her instructors of absence due to illness or injury. Faculty may verify a student's visit to Health Services by phone. Student medical records are confidential. Notification of others, including parents, is generally considered to be the responsibility of the student and will not be usurped unless the condition is serious or the student is unable to assume responsibility for informing others.
Permission will be obtained before an individual's medical information is sent from one health care provider to another.
