If you are from a CCIS member school you must have approval from your study abroad office on your application authorizing your participation in the program. Therefore, you cannot apply on-line. Please download the application below and have it signed and mailed by your study abroad advisor to our office.
If you are under 18 years of age, a parent/guardian signature is required at the end of the application for your enrollment in a study abroad program. Please download the application and mail it to the address below.
If you are not from a CCIS member school and you are over 18 years of age, you can fill out the Study Abroad Online Application.
NOTE: Whether applying on-line or by mail, in order to complete your study abroad file you must download the instructions, appropriate checklist, two recommendation forms, and a course form if you are applying to Australia or Ireland.
